I'm not a PCP (Primary Care Provider) and I'm not the ER. In a sense, I’m everything in between - so chances are, if you have a concern I can help you solve it.

There are add-on charges for procedures & in-home tests as well as for house calls, holidays / after-hours, and very extended discussions / advocacy type work. I am completely transparent about these additional costs, and as a member, you get a discount!
Yes! After an annual $100 Administrative fee (which also covers basic questions), I offer the same services but at a higher rate, and because members get unlimited virtual care, most choose membership. See my Member vs Non-Member prices here.
No. Personalized Urgent Care is a cash only practice. And because I have opted out of Medicare, it is prohibited by law for you or me to seek reimbursement from Medicare.
Most employees are able to use HSA funds for individual and membership fees, but you should check with your Employee Benefits Manager to be sure.
My direct line to call/text in my secure patient platform is 916-347-4468.
Texting via my secure platform is my preferred method. I always respond as quickly as possible between 9am-9pm.
Call me for your most urgent concerns or to wake me up in the middle of the night by calling twice in a row if I don't see your text.
Emailing: Give me up to 3 business days to answer emails for more detailed questions that don’t need immediate attention. I have patient secure emailing which goes directly to me at MD@personalizedurgentcare.com

Yes. You must be a CA resident. I CAN treat you while you are traveling.
I can ALSO see non-CA patients who are visiting and currently in CA.
Short answer: I see all ages. Pregnant, young, old, everyone in between. That is the benefit of my training as a Family Medicine physician.

These are all available and ordered electronically. Often at discounted rates for members. I also offer many rapid tests I can perform in your home.
Yes! Email me a photo of your form to make sure I can meet the requirements, and I will come right to your home! Prices are here, and there is a travel fee.
I also do annual wellness physicals.
My goal is to keep you healthy and be there for you by staying healthy myself, so I have stringent hygiene protocols, carry sanitizer, and often wear a mask. If you or your child is uncomfortable with anything please let me know! (Ex. we can be outdoors to minimize exposure as an option.)
Situations change. Not a problem at all. I just ask you to give me 30 days' notice to tie up any loose ends. If you pay quarterly or annually, you will be immediately refunded the remaining balance. No hard feelings whatsoever.
